I hereby apply to be allowed to use the name of Muhammad Ali as my ring boxing name and request permission that all and any advertising or promotion concerning a proposed bout scheduled for August 6, 1966 in London with Brian London allow any and all people properly connected with the promotion of this bout to use the name of Muhammad Ali.
